Your Initial Consultation

Your initial visit is a comprehensive evaluation and consultation, which will allow Jay P Malmquist, DMD or Michael P Malmquist, DMD to determine the treatment plan that will best meet your individual needs. Initial Implant, TMJ, or Ortho consultation fees will be applied towards any treatment plans.

To assist Drs. Jay and Michael Malmquist in determining the optimal treatment plan for you, please bring the following to your initial consultation:

  • The surgical referral form completed by your dentist
  • Any current radiographs
  • A list of the medications you are allergic to or are presently taking

The clinical examination will consist of a visual exam and a radiographic evaluation with the necessary diagnostic radiographs. If your treatment needs are complex, Drs. Jay and Michael Malmquist may prescribe additional diagnostic records, such as three-dimensional imaging or models of your teeth. Once he has reviewed all of the findings from the clinical exam and diagnostic records, Drs. Jay and Michael Malmquist will present your optimal treatment plan.

They will explain the procedures involved, as well as the treatment sequence, and answer any questions you may have. They will also show you photos of similar cases and computer images of your proposed treatment plan so that you can better understand their recommendations.

Following your consultation, one of our Treatment Coordinators will be available to assist you with scheduling appointments and financial issues. If your treatment plan involves other dental specialists, she will coordinate the treatment sequence with the other offices for you and will serve as your personal liaison with them.

If you have medical insurance or dental benefits, please bring the necessary completed forms, as well as the booklet that outlines your specific plan. This will enable us to expedite the processing of your claims.

Important: All patients under 18 years of age should be accompanied by a parent or guardian at the consultation appointment.
